BackgroundCancer patients often suffer from psychological distress during or after cancer treatment, but the use of psycho-oncological care among cancer patients is limited. One of the reasons might be that the way psycho-oncological care is organized, does not fit patients’ preferences. This study aimed to obtain detailed insight into cancer patients’ preferences regarding the organization of psycho-oncological care.Methods18 semi-structured interviews were conducted among cancer patients. Patients completed psycho-oncological treatment between 2015 and 2020 at the psychology department in a general hospital (...) or a center specialized in psychological cancer care in the Netherlands. The interview comprised questions related to preferences regarding the institute where to receive treatment, the psychologist who provides treatment, and the type of treatment, as well as questions related to experienced barriers and facilitators to receive psycho-oncological care. Interviews were digitally recorded and transcribed verbatim. Data were analyzed individually by two coders into key issues and themes.ResultsRegarding the institute, easy accessibility and prompt availability of psychol-oncological care were considered important. Regarding the psychologist, most participants had a strong preference to be treated by a psychologist specialized in cancer or other somatic diseases. Individual face-to-face therapy was preferred above other types of treatment. Several barriers were mentioned to receive psycho-oncological treatment, among which poor accessibility to psycho-oncological care, lack of knowledge on the possibilities for psycho-oncological treatment, and stigma. Most frequently mentioned facilitators were being assertive to ask for help, having a good relationship with the healthcare professional, and the integration of psycho-oncological support within medical cancer care.ConclusionFrom the patient’s perspective, the organization of psycho-oncological care for cancer patients should focus on easy accessibility and availability, delivered by specialized psychologists, and integration in medical cancer care. Online and group therapy are acceptable, but individual face-to-face therapy is preferred. It is warranted to increase awareness on psycho-oncological care targeting both patients and healthcare providers. (shrink)

ABSTRACT This paper argues for the contemporary relevance of melancholy as something different than depression or a state of mental illness. Instead, through examples of a literary, philosophical, and artistic nature, it is shown that melancholy functions as a force-field – a topos where the finite and the infinite, the earthly and the heavenly, the physical and the spiritual come together and meet in huge tension. By means of an exploration of the historical notion of melancholy and a revisit of (...) Dürer’s Melencolia I (1514), coupled to a much more recent example, Lars von Trier’s film melancholia (DK/se/fr/de 2011), this paper shows how the melancholic motif continues to play an essential role in our contemporary society. The melancholic is described as a person who lives a life on the border: who intuits the point where two worlds meet and who longs for a revelation of the beyond – an apocalypse (literally: ‘an uncovering’). It is this longing – a nostalgia for being – that increasingly pervades our contemporary world, a world that represses the heterogeneous through numerous social and political conditions. The study of melancholy here undertaken therefore provides a valuable contribution to the understanding of melancholy in our (post-) modern time. (shrink)

We study the extension of public announcement logic PAL by public assignments, which we call PALA. Just as in the case of PAL, the standard procedure for deciding PALA validity, i.e. the use of so-called reduction axioms to translate PALA formulae into formulae in epistemic logic EL, may lead to exponential growth. In this paper, we show that such a price is not mandatory, for we provide a polynomial translation of PALA into EL. This is based on abbreviations of subformulae (...) by new propositional letters. Such optimal translation also enables us to show the computational complexity of the problem of deciding PALA validity, which turns out to be coNP-complete in the single-agent case and PSPACE-complete in the multiagent case. Public announcement logic is an extension of multiagent epistemic logic with dynamic operators to model the informational consequences of announcements to the entire group of agents. We propose an extension of public announcement logic with a dynamic modal operator that expresses what is true after any announcement: after which , does it hold that Kφ? We give various semantic results and show completeness for a Hilbert-style axiomatization of this logic. There is a natural generalization to a logic for arbitrary events.

Research questions and backgroundThis study explores a highly controversial issue of medical care in Germany: the decision to withhold or withdraw mechanical ventilation in critically ill patients. It analyzes difficulties in making these decisions and the physicians’ uncertainty in understanding the German terminology of Sterbehilfe, which is used in the context of treatment limitation. Used in everyday language, the word Sterbehilfe carries connotations such as helping the patient in the dying process or helping the patient to enter the dying process. (...) Yet, in the legal and ethical discourse Sterbehilfe indicates several concepts: (1) treatment limitation, i.e., withholding or withdrawing life-sustaining treatment (passive Sterbehilfe), (2) the use of medication for symptom control while taking into account the risk of hastening the patient’s death (indirekte Sterbehilfe), and (3) measures to deliberately terminate the patient’s life (aktive Sterbehilfe). The terminology of Sterbehilfe has been criticized for being too complex and misleading, particularly for practical purposes. Materials and methods An exploratory study based on qualitative interviews was conducted with 28 physicians from nine medical intensive care units in tertiary care hospitals in the German federal state of Baden-Wuerttemberg. The method of data collection was a problem-centered, semi-structured interview using two authentic clinical case examples. In order to shed light on the relation between the physicians’ concepts and the ethical and legal frames of reference, we analyzed their way of using the terms passive and aktive Sterbehilfe. Results Generally, the physicians were more hesitant in making decisions to withdraw rather than withhold mechanical ventilation. Almost half of them assumed a categorical prohibition to withdraw any mechanical ventilation and more than one third felt that treatment ought not to be withdrawn at all. Physicians showed specific uncertainty about classifying the withdrawal of mechanical ventilation as passive Sterbehilfe, and had difficulties understanding that terminating ventilation is not basically illegal, but the permissibility of withdrawal depends on the situation. Conclusions The physicians’ knowledge and skills in interpreting clinical ethical dilemmas require specific improvement on the one hand; on the other hand, the terms passive and aktive Sterbehilfe are less clear than desirable and not as easy to use in clinical practice. Fear of making unjustified or illegal decisions may motivate physicians to continue (even futile) treatment. Physicians strongly opt for more open discussion about end-of-life care to allow for discontinuation of futile treatment and to reduce conflict. (shrink)

At the beginning of 2020, COVID-19 became a global problem. Despite all the efforts to emphasize the relevance of preventive measures, not everyone adhered to them. Thus, learning more about the characteristics determining attitudinal and behavioral responses to the pandemic is crucial to improving future interventions. In this study, we applied machine learning on the multi-national data collected by the International Collaboration on the Social and Moral Psychology of COVID-19 (N = 51,404) to test the predictive efficacy of constructs from (...) social, moral, cognitive, and personality psychology, as well as socio-demographic factors, in the attitudinal and behavioral responses to the pandemic. The results point to several valuable insights. Internalized moral identity provided the most consistent predictive contribution—individuals perceiving moral traits as central to their self-concept reported higher adherence to preventive measures. Similar was found for morality as cooperation, symbolized moral identity, self-control, open-mindedness, collective narcissism, while the inverse relationship was evident for the endorsement of conspiracy theories. However, we also found a non-negligible variability in the explained variance and predictive contributions with respect to macro-level factors such as the pandemic stage or cultural region. Overall, the results underscore the importance of morality-related and contextual factors in understanding adherence to public health recommendations during the pandemic. (shrink)

Sublimation is usually defined as a defense-mechanism that desexualizes the sexual instincts. This desexualization then results in socio-cultural activities and psychic health. That means that sublimation is a crucial concept for psychoanalytic thinking, because it seems to connect the Freudian metapsychology with both applied psychoanalysis and psychoanalytic therapy. However, in this article I argue that within Freud's theory sublimation is an empty and redundant concept. It is a redundant concept as far as it 'explains' the socio-cultural tendencies of human beings, (...) since these tendencies can be easily understood as the outcome of other defense-mechanisms, such as repression and identification. For that matter, Freud's only example of the sublimation process — found in Leonardo da Vinci and a Memory of his Childhood — is in fact no sublimation, but a classic example of repression followed by symptom-formation. Secondly 'sublimation' is an empty concept with regard to the psychoanalytic therapy, because in that context it is used to explain something that simply never happens, at least according to Freud, namely a perfect recovery and a completely 'terminable' analysis. These notes contain an annotated edition of the only four extant letters of Burchard de Volder to Philipp van Limborch. In the first letter De Volder provides Van Limborch with some information about the subscription to the Dordrecht Confession of Faith by professors. In the second letter De Volder comments upon Van Limborch’s De veritate religionis Christianae. This letter is interesting as it provides insights into De Volder’s views on religion and theology. The third letter served as a cover letter (...) for De Volder’s sending to Van Limborch a copy of the honestum testimonium on Jacobus Arminius that was requested by Arminius’s widow in 1611. In the fourth letter Volder comments upon the visit that Pieter Burman had paid him. The visit was an episode in the quarrel between Pieter Burman, his brother Frans Burman jr., and Van Limborch, caused by Van Limborch’s remark, in his Theologia Christiana, that Frans Burman sr. had used Spinoza’s words while treating the issue of divine omnipotence in his Synopsis theologiae. Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

Johann Christoph Sturm’s natural philosophy, with which Leibniz engages in “De ipsa natura”, as well as Petrus van Musschenbroek’s epistemology, constitute important steps in the process of the speciation of physics. In this case, speciation is understood as the process through which the explanation of natural phenomena via empirical regularities comes to define the whole domain of the newly established niche of physics, to the exclusion both of teleology and efficient causality.

Nous abordons ici l’étude de la phénoménologie des objets visuels à partir de comptes rendus verbaux, de réponses à des questions ou de descriptions spontanées, ainsi que d’associations libres. Nous demandons même parfois aux sujets de réaliser de simples croquis. Cet éventail de méthodes permet de sonder la structure profonde de la conscience visuelle. Celle-ci est avant tout révélée par ce qui n’est pas spontanément mentionné par des qualités accidentelles ou encore par des changements induits lors de variations ou ajouts (...) mineurs. Nous avons trouvé une concordance remarquable parmi un grand nombre de participants. Ceci démonte que les comptes-rendus verbaux permettent d’explorer puissamment la structure de la conscience visuelle. Tout comme le langage, les « objets visuels simples » ont des racines profondes dans la conscience. Ces fondations « invisibles » servent à définir des figures simples, un « carré » par exemple, comme des objets prototypiques et uniques. La nature des « objets visuels » implique ainsi l’existence d’un « vaste domaine invisible ». Sans une telle prise en compte, la compréhension correcte de la nature de l’objet visuel n’est pas possible. Cette découverte devrait permettre de mener à une théorie de la syntaxe formelle des formes visuelles fondamentales. (shrink)

Freud's anthropology is in fact little more than an amplified psychiatry. For Freud, the human being is in essence a sick animal. In this paper the author discusses why Freud made this so-called 'anthropological turn'. First it is shown that Freud wanted his psychoanalytic theory to be a 'Philosophy of Man'. Secondly it is argued that this can only be the case if the determinants of pathology, that psychoanalysis claimed to have discovered, are constitutive of human subjectivity. This means that (...) the defensemechanisms and the partial sexual instincts interact in each of us and do so necessarily. Although Freud usually merely postulates the necessary character of this interaction, which always produces symptoms, one can nevertheless find some argumentation for this relation distributed in his work. The third part of this paper consists in an enumeration and explanation of the reasons Freud gives for the fact that we can never satisfy our sexual instincts and always have to rely on the substitutes he calls symptoms. Some of these reasons can be led back to the conflict between sexuality and culture, but most of them are determined in an organic way. If this Freudian intuition is true, the 'Philosophy of Man' should be replaced by the 'Philosophy of Psychopathology'. (shrink)
